Admissions

For most patients, the admissions process begins at the Patient Registration Department. Preston Memorial Hospital requires demographic, financial information, and may ask for co-payment at the time of registration, as well as general consent and insurance authorizations for all Emergency Department patients, and inpatient and outpatient admissions. For outpatient services that are not covered by insurance a deposit of at least $25.00 may be required.

The Patient Registration Department is located on the first floor of the hospital and operates 24 hours a day. Look for the “Information” sign.

Patients under the age of 18 must be accompanied by a parent or legal guardian when registering for services at PMH.

Upon your arrival to the Patient Registration Department, please remember to bring the following items:

  • Social Security card





  •  


  • Picture ID





  •  


  • All active insurance cards





  •  


  • Living will, if you have one





  •  


  • Physician orders

    Pre-Registration: A convenient way to register for outpatient care or services at PMH is through the Pre-Registration process, which enables patients to register over the telephone, rather than coming to the hospital. Pre-Registration operators are available 9:00 a.m. to 4:00 p.m., Monday through Friday. For those who use this service, please remember to bring your insurance card(s) and physician’s order on the day of your appointment.
