The Medical Records Department maintains patient health information records. Patient information is available for:
use in patient care
release of information with patient consent for billing, insurance and continued health care

o Receive Your Medical Records:
Before the hospital can release medical records, an "Authorization to Release Information" form must be signed by the patient or his/her legal representative outlining specifically what is to be released and to whom. If the patient is under the age of 18, a parent may sign for a copy of the records in most circumstances. An authorization is valid for 60 days and must be completed each time records are requested.
The Authorization to Release Information can be obtained by visiting the Medical Records department between 8 a.m. to 4:30 p.m., Monday through Friday. You also may request that the form be mailed to you by calling (304) 329-1400 ext. 246. We will make every effort to provide your medical information as quickly as possible. However, keep in mind that most requests take several days to complete. Please try to request your information several days before you need your records.
**There may be a fee for copying your records as follow: Processing/Retrieval $10.00 plus 75 cents per page.
