Medical Records 
The Medical Records Department maintains all patient health information records. Patient information is available for:

release of information with patient consent for billing, insurance and further medical care |

|
| 
providing healthcare givers with past medical history |
How to Obtain Your Medical Records:

Before the hospital can release medical records, an "Authorization to Release Information" form must be signed by the patient outlining specifically what is to be released and to whom. If the patient is under the age of 18, a parent can sign for a copy of the records. An authorization is valid for 60 days and must be completed each time records are requested. |

The Authorization to Release Information can be obtained by visiting the Medical Records department between 8 a.m. to 4:30 p.m., Monday through Friday. You also may request that the form be mailed to you by calling (304) 329-1400 ext. 117. We will make every effort to provide your medical information as quickly as possible. Keep in mind, however, that most requests take several days to complete. Please make sure that your request is made several days before you need your records. |
