Patient Satisfaction Survey
Hospital Quality Alliance: Improving Care through Information
Hospital Critical Access Patient Satisfaction (HCAHPS) Survey
Surveys are sent home with each inpatient and random Emergency Department patients. When these surveys are returned the Quality Improvement Department reviews it and tabulates the responses into the HCAHPS database.
If there are negative comments that need to be investigated the QIS Director sends the comments to the appropriate department manager to continue the investigation. Manager responses are sent back to the QIS Director to review and, if requested, will contact the patient.
The favorable comments are forwarded to the appropriate manager for review.
The patient comments are finally grouped by department and shared with administration and the appropriate committee for their review and offer us the insight to any necessary training or education for our staff.
This information is then scored and compared against other Critical Access Hospitals nationally and in the state of West Virginia to see how we are doing. This allows the hospital to make improvements where needed.
The survey addresses communication with the nurse, the doctor, the overall hospital staff, treatment, and communication of pain management, and about the patient’s medications.
It also addresses the patient’s impression of the cleanliness and quietness of the hospital and whether they would recommend the hospital to others.
This process is used to further improve patient care and satisfaction at Preston Memorial Hospital.
Utilization Review
This portion of Quality Services focuses on the inpatient. The Utilization nurse coordinates the admission, concurrent and retrospective review of all payors, utilizing appropriate criteria. Once the UR nurse reviews the patient’s admission criteria, for example, the doctor’s orders, nurses notes, and any laboratory/radiology reports she then notifies the insurance company to obtain approval for the stay.
The UR nurse integrates these activities with financial and statistical reports. The UR nurse communicates and serves as a resource to physicians and the physicians office staff. The UR nurse interacts and communicates with hospital departments, external agencies and review organizations in order to assist with reimbursement for inpatient stays.
The UR nurse also attends Continuum of Care meetings twice a week to discuss and evaluate any anticipated home care needs and prepare the patient for discharge.
Credentialing and Review Coordinator
The Credentialing coordinator facilitates the Medical staff credentialing process via appointment and reappointments. The credentialing coordinator accepts new physician applications, completes background checks, and verifies references and schools of all physician applicants.
The coordinator informs the QIS director and Peer Review committee of all potential applicants and of those physicians up for reappointment.
Social Work
The Social Worker is available to help patients and their families prepare for the hospital stay, the discharge, and make possible the most effective use of healthcare services. The social worker helps to organize in home care or placement in an extended care facility. They provide information and referrals for community resources.
