The main function of the social workers at Preston Memorial Hospital is to serve as discharge planners to help ensure that patients have the support and resources to return home safely, or help patients with transferring to a nursing home or long-term care facility. They also assist patients with advance directives such as living wills and durable power of attorney, community resources and referrals, assure that continuing needs are met, identify and assist with home care needs and provide support for psychosocial needs.
